2009 Volunteer Water Quality Monitoring Introductory Workshop Online Registration Form

 

The information you include in this form will be emailed to Priscilla Stotts, Volunteer Monitoring Program, Missouri Department of Natural Resources.  This is not a secure form.  If you do not wish to transmit this information over the internet, print out a copy of the paper form and mail it to the address at the bottom of that form.

Please complete an application form for each person wishing to attend.  Due to the high demand and limited space for these classes, individuals under the age of 18 need to be accompanied by a sponsoring adult in order to attend.

If a workshop is marked "FULL/CLOSED" you may register to have your name added to the waiting list for that workshop.  You are NOT enrolled in the class unless you receive a confirmation letter in the mail.
